What You'll Need
- Insurance card — front and back (if using insurance)
- Photo ID — driver's license or state ID
- Current medications — names, dosages, and how long you've been taking them (including supplements)
- Pharmacy information — name and address of your preferred pharmacy
- Previous medical records — if available, any prior psychiatric evaluations or treatment summaries
- Emergency contact — name, phone number, and relationship
What the Intake Packet Includes
Patient Demographics & History
Basic personal information, emergency contacts, insurance details, medical history, current medications, psychiatric history, family mental health history, and social history.
Clinical Screening Tools
Brief, validated questionnaires that help your provider understand your symptoms before your first session. These may include:
- PHQ-9 — Depression screening (9 questions)
- GAD-7 — Anxiety screening (7 questions)
- ASRS — ADHD screening (6 questions)
These are standard clinical tools — your answers help your provider prepare a more focused and productive first appointment.

Frequently Asked Questions
When should I complete my intake forms?
Please complete them at least 24 hours before your first appointment. This gives your provider time to review your information and prepare for a more productive session.
How long does it take?
Most patients complete the full intake packet in 15-20 minutes. You can save your progress and return to it if needed.
Is my information secure?
Yes. Our patient portal is fully HIPAA-compliant with end-to-end encryption. Your information is protected by the same security standards used by hospitals and major healthcare systems.
I don't have all my medical records. Can I still complete the forms?
Absolutely. Fill out what you can — your provider will work with whatever information you have. You can always update your records later through the patient portal.
