Understanding OCD
Obsessive-Compulsive Disorder (OCD) is a chronic condition affecting approximately 2.5% of adults. It's characterized by intrusive, unwanted thoughts (obsessions) and repetitive behaviors or mental acts (compulsions) performed to reduce the anxiety those thoughts cause. OCD is not about being "neat" or "organized" — it's a debilitating condition that can consume hours of every day.
OCD is frequently misunderstood and underdiagnosed. The average delay between symptom onset and proper treatment is 14-17 years. Many people suffer in silence because they feel ashamed of their thoughts or don't realize what they're experiencing is a treatable medical condition.
With proper treatment — ERP therapy and/or medication — 60-80% of patients achieve significant symptom reduction. You don't have to live controlled by intrusive thoughts and compulsive rituals.
Common Types of OCD
OCD manifests in many forms. Some common themes include:
- Contamination OCD: Fear of germs, dirt, or illness — excessive hand-washing, cleaning, avoidance
- Harm OCD: Unwanted thoughts about harming yourself or others — checking, reassurance seeking
- Symmetry/ordering: Need for things to be "just right" — arranging, counting, evening up
- Religious/moral OCD (scrupulosity): Excessive guilt about sins, blasphemous thoughts, moral perfectionism
- Relationship OCD: Constant doubt about your partner, love, or commitment
- Pure O: Primarily obsessional OCD with mental compulsions (rumination, mental checking, reassurance seeking)

ERP Therapy (Exposure and Response Prevention)
ERP is the gold-standard psychotherapy for OCD:
- Exposure: Gradually facing feared thoughts, images, and situations in a structured, supportive way
- Response Prevention: Resisting the urge to perform compulsions, allowing anxiety to naturally decrease
- Hierarchy building: Creating a personalized fear ladder from least to most challenging
- Habituation: Over time, the anxiety triggered by obsessions weakens as your brain learns the feared outcome doesn't occur
Medication Management
OCD typically responds to higher doses of SSRIs than depression:
- Fluoxetine (Prozac) — FDA-approved for OCD, effective at 40-80mg
- Fluvoxamine (Luvox) — specifically developed for OCD
- Sertraline (Zoloft) — FDA-approved for OCD, well-tolerated
- Clomipramine (Anafranil) — the most potent anti-OCD medication (tricyclic antidepressant)
- Augmentation: Low-dose aripiprazole or risperidone may be added for treatment-resistant OCD
Unlike depression, OCD medications typically require 8-12 weeks at full dose before reaching maximum benefit. Don't give up too early — and expect your provider to increase the dose gradually over several weeks.
